In Unani medicine, Mizaj —often translated as temperament—is frequently reduced in modern explanations to a kind of fixed biological label. A person is described as “hot and dry” or “cold and moist,” as if this were a permanent identity stamped onto the body.
But classical physicians like Ibn Sina (Avicenna) present a far more dynamic picture. Temperament, in its original philosophical and medical sense, is not a static classification. It is a continuously adjusting state of balance—alive, responsive, and always in motion.
If we translate this into a modern conceptual frame, the human body is not a fixed condition. It is better understood as a constantly shifting equilibrium system, negotiating heat, cold, moisture, and dryness in real time.

A major misunderstanding in simplified teaching is the idea that temperament is permanent:
> “This person is hot and dry. That person is cold and moist.”
While classical Unani texts do describe dominant tendencies, they also imply something more subtle: the body never remains locked in a single condition.
Even the concept of Hararat-e-Ghariziyya (innate heat) in Ibn Sina’s work is inherently dynamic. It fluctuates based on digestion, physical activity, sleep, emotional states, environmental exposure, and age.
So temperament is not a rigid identity. It is a baseline tendency that continuously oscillates around a moving point of balance.

The body does not remain fixed at the center of this line. Instead, it shifts slightly throughout the day.
After eating, metabolic activity increases and digestion generates heat, nudging the system toward warmth. During sleep, metabolic processes slow down, leading to a cooling and conserving effect. After physical exertion, heat rises more strongly. During rest and calm, the system gradually returns toward baseline.
What appears as “stability” is actually a continuous wave of micro-adjustments around a central equilibrium.
🔹Moisture and Dryness: The Silent Partner Axis
Heat and cold are only one part of the system. Moisture (ratubat) and dryness (yubusat) form the other essential dimension.
Every shift in temperature affects fluid balance. Exercise increases heat and consumes fluids, increasing dryness. Hydration increases moisture and produces a cooling effect. Digestion temporarily uses internal moisture for metabolic processing.
Thus, the body is always balancing a two-dimensional system, not a single scale.
🔹Continuous Influence of the Six Essential Factors
Unani medicine describes six essential lifestyle influences (Asbab-e-Sitta Zarooriya) that continuously shape temperament. These are not occasional factors—they are ongoing forces.
Food is one of the most direct influences. Spicy or rich foods increase heat, while watery or light foods increase moisture. Even after eating, digestion continues to shift internal balance.
Environment is equally constant. Cold air gradually reduces heat, while warm climates increase it over time. Humidity affects moisture retention continuously.
Movement generates heat, while rest conserves it. Sleep restores moisture and cools the system, while wakefulness increases metabolic activity and fluid consumption.
Emotions also play a direct physiological role. Anger produces heat, fear causes internal withdrawal and cooling, and sadness tends toward depletion and reduced vitality.
Finally, processes of retention and elimination—digestion, sweating, urination, respiration—ensure that balance is constantly being adjusted through removal and replenishment.
🔹The Hidden Intelligence of the Body: Tabi’at
If everything is constantly changing, why doesn’t the body fall into disorder all the time?
Classical Unani theory answers this with the concept of Tabi’at—the body’s innate self-regulating intelligence.
This is not mystical in nature. It describes the biological tendency of living systems to correct imbalances automatically. When heat rises excessively, the body activates cooling mechanisms. When dryness increases, fluid conservation and restoration processes are triggered. Sleep, digestion, excretion, and circulation all participate in this continuous correction.
In this sense, the body is not passive. It is actively engaged in maintaining equilibrium at every moment.
🔹Health as Controlled Fluctuation
From this perspective, health is not stillness or rigidity. It is controlled movement within safe boundaries.
The body is always fluctuating, but in health these fluctuations remain within a manageable range and return naturally to baseline.
Disease begins when this pattern is disrupted—when fluctuations become too extreme, when recovery becomes slow, or when regulatory strength is weakened.
🔹Individual Temperament as a Pattern, Not a Label
Every person has a natural baseline temperament, but this is not a fixed condition. It is more accurately a reference center around which variation occurs.
A naturally hot individual still experiences cooling states through sleep, fasting, or environmental exposure. A naturally cold individual still generates heat through activity, digestion, or emotional arousal.
The difference lies not in whether change occurs, but in how the system returns to balance.
Individuality, therefore, is not about static classification. It is about the pattern of regulation.
🔹A Practical Shift in Thinking
When this framework is applied in practice, the focus shifts fundamentally.
Instead of asking, “What is this person’s temperament?” the more useful question becomes:
“What is currently pushing this system out of balance?”
This opens the door to dynamic analysis—diet, sleep, emotional stress, environmental exposure, and physiological state all become active variables rather than fixed labels.
Treatment, in this sense, is not about imposing change from outside. It is about supporting the body’s own regulatory intelligence so it can return toward equilibrium more effectively.
