🔰Bridging Tissue Immunology with Tibb-e-Unani: From “Blood” to the Diseased Tissue


Modern immunology is making a profound shift: the bloodstream does not tell the whole story of disease.

Tissues possess their own immune microenvironments—containing tissue-resident T cells, macrophages, stromal cells, extracellular matrix and local signalling networks. These can generate highly localized inflammatory responses even when systemic parameters appear relatively unremarkable.

Interestingly, this opens a powerful conceptual dialogue with Tibb-e-Unani.

Unani pathology does not reduce disease to circulating blood alone. It recognizes:

🔹 Mizaj-e-A'za — organ/tissue-specific temperament
🔹 Sue-e-Mizaj-e-A'za — localized dystemperament
🔹 Sue-e-Mizaj Maaddi — pathology associated with morbid matter
🔹 Waram — localized inflammatory pathology
🔹 Tabi'at / Quwwat-e-Mudabbira — the body's regulatory and restorative capacity
🔹 Asbab-e-Sittah Zarooriyah — environmental, dietary, behavioural and psychological influences

This creates an intriguing Modern–Unani bridge:

Circulating immune state → Akhlat / systemic milieu
Tissue microenvironment → Mizaj-e-A'za
Local immune response → Tabi'at + Waram dynamics
Environmental exposure → Asbab
Persistent tissue dysfunction → Sue-e-Mizaj-e-A'za

Modern science now demonstrates that immune cells can become highly specialized according to the tissue in which they reside; tissue macrophages and resident lymphocytes perform organ-specific functions and respond to local signals.

Unani's concept of Mizaj should not be claimed as a direct historical equivalent of tissue immunology. But it offers a valuable conceptual framework for asking a modern clinical question:

Is the disease really in the blood—or is the blood merely reflecting a deeper tissue-level disturbance?

This may be especially relevant to chronic inflammatory disorders of the skin, gut, liver, respiratory tract and joints, where local tissue–immune interactions can sustain disease.

The future of Applied Unani Medicine may therefore lie not in replacing modern immunology, but in developing testable bridges between Mizaj-e-A'za, tissue microenvironment, local inflammation and systemic biomarkers.

From “What is abnormal in the blood?”
to “What is happening inside the tissue?”

That is where a truly Applied Unani Immunopathology could begin.
