🧠 What if brain ageing and brain failure are not the same thing?
Modern neuroscience has revealed a remarkable phenomenon: some individuals can show substantial age-related or pathological changes in brain structure while maintaining relatively preserved cognition and daily function. This capacity is often discussed through concepts such as cognitive reserve, compensation and neuroplasticity.
Interestingly, this perspective resonates with several foundational ideas of Unani Hikmat (Greco-Arab Medicine).
🔬 An Applied Unani Perspective
1️⃣ Tabiyat-e-Mudabbira-e-Badan — The Regulatory Intelligence
Unani medicine places Tabiyat at the centre of physiological regulation and adaptation. Rather than viewing health simply as the absence of structural pathology, it emphasizes the body's continuing ability to preserve function and restore equilibrium.
In modern terms, this invites an interesting research question: could individual differences in adaptive capacity be conceptually compared with cognitive reserve and neural compensation?
2️⃣ Mizaj — Function Beyond Structure
Unani thought does not interpret disease solely through anatomical change. Mizaj represents the functional quality of the organism, while disturbances in qualities such as Burudat (coldness) and Yubusat (dryness) were traditionally considered relevant to disorders involving neurological function.
This provides a classical framework for asking why similar structural changes may produce very different functional outcomes in different individuals.
3️⃣ Quwa Nafsaniyya — Preservation of Mental Faculties
Memory, cognition, perception and other higher functions were understood within the broader concept of Quwa Nafsaniyya. Their preservation was linked not merely to anatomy, but to the integrated functioning of the organism.
4️⃣ Hifz-e-Sehat & Asbab-e-Sitta Zarooriya
Perhaps the strongest bridge with preventive neuroscience lies in Unani's emphasis on maintaining health before disease becomes disabling—through regulation of sleep and wakefulness, physical activity and rest, nutrition, mental states, environmental exposure and elimination.
💡The Research Opportunity
The future may not be limited to asking “How much structural damage is present?”
A more meaningful question may be:
“How effectively can the individual preserve function despite biological ageing or pathology?”
This is where cognitive reserve, neuroplasticity and Unani concepts of Tabiyat, Mizaj and Hifz-e-Sehat can enter a fascinating interdisciplinary research dialogue.
The goal is not to equate classical concepts with modern neuroscience, but to critically investigate where they converge, where they differ, and whether Unani theory can generate testable hypotheses for healthy brain ageing.
