🔰 MUFARREHAT: FROM “MOOD-LIFTING DRUGS” TO PSYCHOPHYSIOLOGICAL SUPPORTIVE THERAPEUTICS

One of Unani medicine’s most sophisticated—but underdeveloped—therapeutic concepts is Mufarrehat.

Traditionally, they are associated with Farah, strengthening of the heart and improvement of the mental state.

❗But can we clinically reinterpret them for the modern patient with multimorbidity?

I believe we should.

Today’s patient may present with:

Chronic stress + dyspepsia + poor sleep + fatigue + palpitations + anxiety-like symptoms + metabolic dysfunction + reduced motivation.

Modern science increasingly describes these not as isolated compartments but as interacting systems. WHO now explicitly promotes integrated approaches linking mental, neurological and physical health.

And contemporary research on the gut–brain axis shows bidirectional communication involving neural, endocrine, immune and microbial pathways; chronic stress can influence gut function, permeability, inflammation and mental health.

THIS OPENS A NEW DOOR FOR UNANI.

Rather than defining Mufarrehat merely as “exhilarants”, I propose a clinically useful working definition:

MUFARREHAT = PSYCHOPHYSIOLOGICAL SUPPORTIVE THERAPEUTICS

Their potential therapeutic target may be the functional state of the patient, not merely the disease label.

AN APPLIED UNANI MODEL

Mizaj → Quwwat-e-Nafsaniya → Quwwat-e-Qalb → Quwwat-e-Hazima → Sleep/Stress → Functional resilience

Thus, a Mufarreh intervention could be considered when the clinical picture suggests:

• diminished Farah / mental vitality
• stress-associated functional symptoms
• convalescent weakness
• appetite–digestion disturbance with emotional overlay
• sleep–wake dysregulation
• stress-related palpitations
• reduced psychological resilience

But there is an essential caveat:

Mufarrehat should NOT simply be renamed “antidepressants”, “anxiolytics” or “adaptogens”.

That would be reductionism.

The real opportunity is to investigate which Mufarreh, in which Mizaj, for which functional phenotype, at what dose and for what therapeutic endpoint.

THE NEXT GENERATION OF APPLIED UNANI RESEARCH

Instead of asking:

“Is this herb antidepressant?”

Ask:

❗“Does this Mufarreh improve Farah, sleep, digestion, autonomic symptoms, stress resilience and functional recovery—and through which mechanisms?”

This creates a potentially powerful bridge between Unani psychophysiology and modern brain–gut–body science.

Mufarrehat may therefore deserve a new research identity:

Not merely “exhilarants.”
Not merely “cardiac tonics.”
But potential PSYCHOPHYSIOLOGICAL SUPPORTIVE THERAPEUTICS.

This could become a new frontier of Applied Unani Medicine.
